Transforming Episode Accountability Model (TEAM) CMS Mandatory Alternative Payment Model

Acute care hospitals paid under the Inpatient Prospective Payment System (IPPS) and located in selected Core Based Statistical Areas (CBSAs) are required to participate in TEAM.

Lower Extremity Joint Replacement, Surgical Hip and Femur Fracture Treatment, Spinal Fusion, Coronary Artery Bypass Graft, and Major Bowel Procedures
Hospital Mandatory Participation
Hospitals required to participate are based on selected geographic regions, CBSAs, from across the United States.
5-Year Model
The model runs 2026 to 2030. Designed with a one-year glide path to mandatory downside financial risk for most hospital participants.
Over 700 hospitals were selected as mandatory participants in TEAM. The model is designed with 3 tracks of financial accountability. All participants can select to participate in an upside only track for year 1. In year 2, most hospitals must move to a downside risk track.
