ASM CMS Office Hours clarified portal onboarding, participant responsibilities, reporting requirements, and unresolved policy questions for the 2027 model start.

Portal Onboarding

  • ASM-selected physicians must register individually through the CMS Enterprise Portal to confirm contact information, complete required onboarding activities, and gain access to the data, tools, and resources needed to succeed in the model.

  • Registration should occur as soon as possible at the participant level. Failure to register for the portal puts a participant at risk of missing model-specific deadlines.  Once the portal account is created, physicians may add organizational contacts who will help support their participation in ASM. Every organization must retain at least one primary contact. However, the individual ASM participant must register for the portal first before any additional contacts can be added.

  • The CMMI Salesforce Help Desk and ASM Help Desk are the main channels for registration and organization-specific issues (AmbulatorySpecialtyModel@cms.hhs.gov)

Participation Rules

  • ASM participation is determined at the individual TIN-NPI level, not automatically for an entire organization. For any year that the TIN-NPI is on the ASM participation list, the physician is exempt from MIPS.

  • Participants remain in the model test period for all five performance years, while annual performance requirements are evaluated separately. Organizational changes may affect participation eligibility for a given performance year.

  • If a physician was included on the ASM participation list using 2025 claims data under a TIN that the physician no longer bills under, the physician must notify CMS using the organizational change request feature in the participant portal.

Reporting Guidance

  • Quality measures are generally all-payer, except administrative claims measures calculated from Medicare data. Improvement activities must be in place for at least 90 consecutive days.

  • Additional guidance on cost measure reporting,  data-sharing, and improvement-activities is forthcoming.
