47 HCCs Across the Bundles - 11 in All Bundles

Did you know there are 47 HCCs across the TEAM bundles—11 appear in all five? 

Under the proposed rule, CMS transitions to HCC Version 28 (v28), the TEAM model’s risk adjustment methodology is also evolving. There are 47 Hierarchical Condition Categories (HCCs) used across the five TEAM procedural bundles—and 11 of those HCCs appear in every bundle. 

Each bundle’s unique mix of HCCs reflects different clinical complexity and cost drivers. Understanding which conditions influence your episode benchmarks is critical to accurate risk capture and target price management.
