In-Kind Patient Incentive

Did you know TEAM allows in-kind patient engagement incentives? 

TEAM participants may offer in-kind items or technology—up to $1,000 per patient per episode—to help improve outcomes. These incentives must support clinical goals such as care plan adherence, complication prevention, and chronic condition management.📌 Important: Anything over $25 must be documented, and items must directly align with TEAM’s defined engagement goals.
