With the new CMS Transforming Episode Accountability Model (TEAM) taking effect next year, hospitals must ensure the right measures are in place to meet the model’s requirements.

Through this research, we are seeking to understand the current outlook on the TEAM initiative and what the overall readiness looks like.

We will have you answer questions around awareness and understanding, previous experience, organizational readiness, and anticipated challenges.

All feedback will be kept confidential and only disclosed in the aggregate.
