CMS Updates ASM Resources and Releases Participant List


CMS has released the Ambulatory Specialty Model (ASM) preliminary participants dataset for the 2027 performance year starting January 1, 2027. The list identifies clinicians required to participate — including NPIs, cohorts, and applicable performance years — with ~2.6K heart failure and ~4.0K low back pain participants currently identified.


Early review is critical to confirm inclusion, understand upcoming requirements, and ensure contact information is updated through the ASM Participant Contact Information Form. Organizations should begin assessing operational readiness now, as final participants will be confirmed later in 2026 — and participation is mandatory for those meeting CMS criteria across selected specialties and geographies.


CMS released a pre-implementation checklist for specialists required to participate in the mandatory ASM. The resource highlights eight key readiness categories clinicians should evaluate now as they prepare for the model’s 2027 launch.
